Disseminated Lyme disease, due to unsuccessful or delayed treatment, can become disabling. The bacteria can leave the skin where it was initially inoculated by the tick and travel through the bloodstream to numerous systems of the body, primarily joints, heart, brain, muscles and the nervous system.
Late disseminated Lyme disease has a wide range of presentations including joint pain, extreme fatigue, neuromuscular pain, cardiac problems, headaches, and other central nervous system dysfunction. There are some distinguishable signs of later stage Lyme disease including facial palsy in the second stage, and swollen knees in the third stage that are somewhat specific for Lyme disease, but not absolutely, because there are other causes of Bells Palsy and swollen knees.

When the skin rash is typical and when a patient has been exposed to an environment where blacklegged ticks are known to be established, the diagnosis can be made on clinical grounds alone. In parts of Canada where adventitious, unestablished populations of blacklegged or western blacklegged ticks have been noted, a clinical diagnosis is more challenging. When the rash is atypical or occurs in circumstances in which exposure to the appropriate vector tick species was unlikely, diagnosis is based on the demonstration of a serological response to B burgdorferi. Immunoglobulin M antibodies are usually detectable within weeks of the onset of symptoms however, a significant proportion of patients with EM may not have detectable antibody at the time of initial presentation . Furthermore, when patients are treated very early in the course of illness, antibodies may not develop. When an initial antibody determination is negative, it is suggested that a second serum specimen be collected four weeks later.
Current evidence suggests that commercially available enzyme immunoassays used for the purpose of screening are sufficiently sensitive .
Antigen detection has also been used in both spinal fluid and urine. As with NAT, antigen tests cannot be recommended unless their sensitivity and specificity significantly improve .
Future Possibilities For Diagnostic Tools
NIAID-supported scientists have identified genome sequences for multiple strains of B. burgdorferi. Greater advances in diagnostics are anticipated as genetic information is combined with advances in microarray technology, imaging, and proteomics. These growing fields of science are expected to lead to improved diagnostic tools as well as provide new insights on the pathogenesis of Lyme disease. Examples of tools being developed with NIAID support include use of metabolomics to characterize new biomarkers of infection, next generation T-cell based measurements, and novel antigens for improved measurement of effective treatment.

If joint lameness is the presenting sign, improvement may be noted after 3-5 days starting antibiotics. Dogs that start with a moderate to high QC6 value typically show a 50% reduction or more in the QC6 at six months, indicating that treatment has been successful. Lyme disease is caused by Borrelia bacteria, and spread by infected ticks. If left untreated, it can cause fatigue and weakness, as well as damage to the heart, nerves and other organs, according to Health Canada.
However, getting a diagnosis can be a battle, said Wilson.
There isn’t an early, reliable blood test to detect Lyme, and symptoms can vary from person to person including skin rash, headache, fever and swollen lymph nodes.
Doctors will factor in whether someone has been in an “at-risk” or “endemic area,” where ticks are known to carry Lyme disease, which according to Health Canada includes:

However, Wilson doesn’t think there hasn’t been enough surveillance to reliably know where infected ticks can be found.
“There’s this overemphasis on ‘known endemic areas,'” said Wilson. “We don’t know where Lyme disease is. It could be anywhere is our mantra.”
That’s why a test that someone could buy at a drug store to check for the Borrelia bacteria could be helpful, despite its limitations, he said.

How Do You Test A Tick For Lyme
Yes, some state and local public health departments offer testing of ticks as well as identification of the types that transmit Lyme disease. You can check with your health department to see if it offers these. However, testing ticks may not always be useful for a few reasons.
Lyme Disease Test Western Blot
Labs performing a Western blot use electricity to separate proteins called antigens into bands. The read-out from the Western blot looks like a bar code. If your blot has bands in the right places, and the right number of bands, it is positive.
The CDC requires 5 out of 10 bands for a positive test result.
