Tick borne disease refers collectively to the various infectious pathogens that can be transmitted via the bite of a tick, often referred to as co-infections. Lyme disease, aka borreliosis, is caused by the infectious agent B. burgdorferi in the United States B. afzellii, B. garinii more commonly seen in Europe.
It has been called the second great imitator, after syphilis. Like syphilis it is a slow growing spirochete. Unlike syphilis, it has the largest genome of any bacteria, and is able to avoid immune detection through various mechanisms. As well, it can re-activate past infections, like ebstein barr or mycoplasma, often thought of non-pathogenic in the immune-competent host.

Not all tick bites require antibiotic treatment, but each do require a careful assessment to determine possible transmission of pathogens. How Many People Get Lyme Disease
Approximately 30,000 cases of Lyme disease are reported to CDC by state health departments each year. However, this number does not reflect every case of Lyme disease that is diagnosed in the United States every year. According to the CDC, since many cases go unnoticed by the patient, may be misdiagnosed, or go unreported to the CDC, the actual number of cases could be higher.
The CDC wants to know how many people are actually diagnosed with Lyme disease each year in order to improve the health and safety of the public, and for this reason they have conducted two studies:
- Project 1 estimated the number of people who tested positive for Lyme disease based on data obtained from a survey of clinical laboratories. Researchers estimated that 288,000 infections occur among patients for whom a laboratory specimen was submitted in 2008.
- Project 2 estimated the number of people diagnosed with Lyme disease based on medical claims information from a large insurance database. In this study, researchers estimated that 329,000 cases of Lyme disease occur annually in the United States.
Results of these studies suggest that the number of people diagnosed with Lyme disease each year in the United States is around 300,000. These estimates do not affect our understanding of the geographic distribution of Lyme disease. Lyme disease cases are densely concentrated in the Northeast and upper Midwest. These 14 states, shown in the map below, account for over 96% of cases reported to CDC.

More Than Lyme: Tick Study Finds Multiple Agents Of Tick
STONY BROOK, N.Y., September 16, 2019 In a study published in mBio, a journal of the American Society for Microbiology, Jorge Benach and Rafal Tokarz, and their co-authors at Stony Brook University and Columbia University, reported on the prevalence of multiple agents capable of causing human disease that are present in three species of ticks in Long Island.
Tick-borne diseases have become a worldwide threat to public health. In the United States, cases more than doubled, from 22,000 in 2004 to more than 48,000 in 2016, according to the U.S. Centers for Disease Control. Tick-borne diseases range from subclinical to fatal infections with disproportionate incidence in children or the elderly. Moreover, some infections can also be transmitted by blood transfusions and cause severe disease in patients with underlying disorders. While public attention has focused on Lyme disease, in recent years, scientists have uncovered evidence that ticks can carry several different pathogens capable of several different tick-borne diseases, sometimes in a single tick.
In evaluating tick-borne infection, more than one organism needs to be considered, says senior author Rafal Tokarz, PhD, assistant professor of epidemiology in the Center for Infection and Immunity at the Columbia Mailman School of Public Health, and a graduate of the Department of Microbiology and Immunology at Stony Brook University. This study emphasizes the need to focus on all tick-borne diseases, not just Lyme.
How Does The Ticks Get This Infection To Transmit To Others

What To Do When Serologic Testing Is Negative But Lyme Is Still Suspected
The problems with the conventional two-tier testing strategy for Lyme disease have initiated the development of new tests from independent labs intended to offer higher sensitivity and specificity.
As the first step in the diagnosis of Lyme, we often recommend serologic tests, including the ELISA, the Western Blot, and the Immunoblot. This new test is more sensitive than the Western Blot and is designed to take its place.38 Serologic Lyme tests can be done through Stony Brook University, Quest, LabCorp, or IGeneX.
If serologic tests come back negative and Lyme is still strongly suspected, we may recommend molecular testing such as the test offered by Global Lyme Diagnostics. The GLD tests antibodies to B. burgdorferi outer surface proteins, expanding the number of species and strains that can be detected.
We may also recommend Elispot T cell activation tests. These tests are run by labs such as InfectoLab and Armin Laboratories, and look at T cell activation for the pathogens. This may be a better indicator of current or recent infection and lead to a more accurate diagnosis.
However, it is important to remember that the testing for Lyme and other tickborne pathogens is still far from perfect, and we always focus on your individual case and symptoms. Lyme disease is primarily a clinical diagnosis and the tests are used as supportive aids.
What Is The Course Of Lyme Disease Treatment
It is easier to treat Lyme disease in its early stages. If your doctor suspects you have Lyme disease, theyll prescribe a course of antibiotics. In case the symptoms are severe, your doctor may refer you to a specialist for injections of antibiotics. If antibiotics fail to work, your doctor will get more blood tests done and monitor your symptoms over a specified time frame.

The results of a phase 1/2 clinical trial in Europe of an investigational Lyme disease vaccine co-developed by researchers at Stony Brook University, Brookhaven National Laboratory, and at Baxter International Inc., a U.S. based healthcare company, revealed it to be promising and well tolerated, according to a research paper published online in The Lancet Infectious Diseases. The vaccine was shown to produce substantial antibodies against all targeted species of Borrelia, the causative agent of Lyme disease in Europe and the United States. Baxter International conducted the clinical trial of the vaccine.
To formulate a Lyme vaccine that is effective on all Borrelia species, researchers bioengineered a set of unique outer surface proteins that share different parts from different strains of the bacteria.
” The results are promising because the vaccine generated a potent human immune reaction, covered the complete range of Borrelia active in the entire Northern hemisphere, and produced no major side effects.”

“After a series of experimentations and refinements, formulations consisting of these new OspA proteins were shown to protect against a broad spectrum of Lyme disease spirochetes,” said Dr. Luft, summarizing the research results.
