Treatment For Chronic Lyme Disease

Sometimes, people go through treatment for Lyme disease but their symptoms donât go away. If this lasts over 6 months, itâs known as chronic Lyme disease or âpost-treatment Lyme disease syndromeâ .

Doctors still arenât sure why some people get PTLDS. Some believe that getting Lyme disease may cause damage to your tissues or immune system. Others believe itâs because the bacteria that causes Lyme hasnât completely gone away.

There is little evidence that taking more antibiotics at this stage will help. They may actually be harmful. Instead, your doctor will focus on treating the symptoms youâre still having. This will be different for everyone. Some people could benefit from a medicine that relieves fatigue, while others may need a drug that can help with headaches or very sensitive skin.

Your doctor could also have you try a treatment that helps people with chronic fatigue syndrome or fibromyalgia.

More research is needed to figure out how best to treat PTLDS. Itâs something that can be frustrating. Just remember: Many people who have this condition do start feeling like their old selves after a few months.

History And Physical Examination

The clinical diagnosis of Lyme disease can be straightforward in patients with a history of tick exposure and the characteristic finding of an erythema migrans rash.12 The CDC has defined erythema migrans rash as an expanding red macule or papule that must reach at least 5 cm in size .11 According to the Infectious Diseases Society of America guidelines, erythema migrans rash is the only clinical manifestation sufficient to make the diagnosis of Lyme disease in the absence of laboratory confirmation.20 Although one study concluded that primary care physicians in a Lyme diseaseendemic area of France correctly identified erythema migrans in 72 percent of patients,22 the study was limited by lack of complete clinical information for the participants.

A number of conditions resemble erythema migrans however, the rapid and prolonged expansion of an erythematous lesion is unique to erythema migrans.11 Lesions most often occur at anatomic sites that are unusual for cellulitis and other conditions that mimic erythema migrans therefore, a complete skin examination should be performed before excluding erythema migrans.20

What Are The Side Effects Of Lyme Disease Treatments

Antibiotics, like all medications, have the potential for side effects. Any antibiotic can cause skin rashes, and if an itchy red rash develops while on antibiotics, a patient should see their physician. Sometimes symptoms worsen for the first few days on an antibiotic. This is called a Herxheimer reaction and occurs when the antibiotics start to kill the bacteria. In the first 24 to 48 hours, dead bacterial products stimulate the immune system to release inflammatory cytokines and chemokines that can cause increased fever and achiness. This should be transient and last no more than a day or two after the initiation of antibiotics.

The most common side effect of the penicillin antibiotics is diarrhea, and occasionally even serious cases caused by the bacteria Clostridium difficile. This bacterial overgrowth condition occurs because antibiotics kill the good bacteria in our gut. It can be helpful to use probiotics to restore the good bacteria and microbiome balance.

How Effective Is Doxycycline For A Tick Bite

Doxycycline is an effective antibiotic used to treat tick bites that cause illnesses. Doxycycline is an antibiotic, specifically a tetracycline derivative and is available as hyclate, monohydrate and calcium salts.

Tick bites can cause diseases and infections such as Lyme disease, Rocky Mountain spotted fever, Colorado tick fever, and cellulitis by transmitting microscopic parasites that infect red blood cells. According to the Centers for Disease Control and Prevention , approximately 30,000 people are diagnosed with Lyme disease each year and 6,000 a year from Rocky Mountain spotted fever.

Single Dose Of Doxy After Tick Bite Only Prevents Rashnot Lyme Disease

The Journal of Emergency Medicine recently published an article entitled Lyme Disease: Emergency Department Considerations. The authors recommend using a one-time, single dose of doxycycline after a tick bite to prevent the onset of Lyme disease, despite the fact that there has been only one study exploring the effectiveness of such a limited dosage. The article also neglects to mention that there are doctors who take a different approach and advise against a one-time, single dose. The following is republished from the All Things Lyme blog.

The authors cite the 2006 Infectious Diseases Society of America guidelines when making their recommendation that individuals be treated with a single dose of doxycycline .

Several Antibiotics Appear Effective Against Early

Most cases of Lyme disease, which is an infection carried by ticks, can be easily managed if treated early using antibiotics, with choice of antibiotic agent having little bearing on success.

This network meta-analysis suggests that when symptoms of the disease are confined to a localised skin infection, treatment failures are relatively infrequent, only 2% at 12 months. Effective antibiotics include penicillin V, doxycycline, azithromycin, cefuroxime, amoxicillin and ceftriaxone plus doxycycline.

The research has significant limitations, but choice, dose and duration of antibiotics appear to have little impact on outcomes. This research does not suggest that guidelines need to be reviewed.

What Did This Study Do

This network meta-analysis included 19 randomised controlled trials involving 2,532 people with erythema migrans due to Lyme disease. No trials were based in the UK most were from the United States or Europe.

Each trial compared two or three antibiotics. Doxycycline was the most frequently used antibiotic. A number of doses and treatment durations were represented. For example, treatment length for doxycycline ranged from 10 to 21 days. For the main analyses, doxycycline was used as the reference treatment.

Both direct and indirect data were extracted from the studies. This approach is helpful when there is a shortage of head to head research, but this, and the overall low quality of included studies means that results need to be treated with some caution.

The best currently available method for preventing infection with B. burgdorferi and other Ixodes-transmitted infections is to avoid tick-infested areas . If exposure to I. scapularis or I. pacificus ticks is unavoidable, a number of measures may help to decrease the risk that ticks will attach and subsequently transmit infection. The use of protective clothing may interfere with attachment by ticks by increasing the time required for ticks to find exposed skin, thus facilitating their recognition and removal. By wearing light-colored clothing , persons in areas of endemicity may also be more likely to see ticks before they have attached.

Daily inspections of the entire body to locate ticks also provide an opportunity to prevent transmission of tick-borne infections .
