Can Lyme Disease Cause Cognitive Dysfunction Or Dementia

The question of whether Lyme disease could cause dementia was addressed by Wormser and colleagues in an article entitled Lack of Convincing Evidence That Borrelia burgdorferi Infection Causes Either Alzheimer Disease or Lewy Body Dementia, published in the journal Clinical Infectious Diseases.¹ The authors reviewed a paper by Gadila et al.² which concluded that a Lyme disease infection might cause Lewy body dementia. Borrelia Burgdorferi Infectious Cycle

The infectious cycle of B burgdorferi involves colonization, infection of Ixodes ticks, and then transmission to broad a range of mammalian hosts, including humans. Variation in environmental and host conditions promotes different gene expression and changes in the composition of the membrane proteins of the spirochete. This adaptation is a critical step in the pathogenesis and transmission of Lyme disease.

The Ixodes tick progresses through four stages of development: egg, larva, nymph, and adult . Only larvae, nymphs, and adult female ticks require blood meals, and only ticks in the nymphal and adult stages can transmit B burgdorferi.

The life cycle of Ixodes ticks spans 2 years . The adult lays eggs in the spring, and the larvae emerge in the summer. The larvae feed once, in late summer, on any of a wide variety of small animals . The following spring, the larvae emerge as nymphs. Nymphs feed once, in the spring and summer. The white-footed mouse is the preferred feeding source of nymphs, but other animals apparently suffice. Nymphs molt into adults the following fall and feed once on a larger animal, with the white-tailed deer being the preferred host.

Ticks carry B burgdorferi organisms in their midgut. The bacteria are introduced into the skin by a bite from an infected tick, and disease is transmitted to humans as the spirochete is translocated from the gut to the salivary glands and then to the person at the site of the bite.

Georgia Lyme Disease Association Participates In Institute Of Medicines Lyme Teleconference

The Institute of Medicine invited Georgia Lyme Disease Association members to participate in a Lyme Disease teleconference call on June 2, 2010. This regional listening session for Georgia/North Carolina/South Carolina and surrounding states was arranged as part of the planning for the State of the Science workshop to be held in October. The call was designed so that IOM Lyme Disease panel members would hear concerns pertaining to issues that should be considered and addressed during the upcoming conference.

Scientists in the Southeastern USA are not awarded funding for independent, large-scale human studies of Lyme and tick-borne disease pathogens, based on our flawed Lyme disease statistics. Our few reported cases make it seem as if Lyme is not a problem here. However, as advocates pointed out, Lyme disease IS a problem here – our cases are not counted and reported using CDC reporting criteria. Many continue to be dismissed as false positives. Lyme is often unrecognized in the Southeast, therefore, patients dont get early treatment that could prevent permanent damage and ongoing symptoms. We called for immediate funding to investigate these cases.

A July 2014 wedding on Long Beach Island, New Jersey was the start of my nightmare.

It was a beautiful weekend filled with love for two of our dearest friends who were, after 14 years together, finally legally able to marry. There was lobster and champagne and a crazy Gordon Gekko-style house on the beach. I sang a few songs, made a toast, and cried happy tears.

I woke up two days later, back home in NYC, with a crushing head and neck ache. It was weird, but I figured I would sleep it off and be better in the morning. It took a few days, but I recovered.

That Saturday, I got out of the shower and saw a faint red circular rash on my shoulder with a bug bite in the middle. My knees buckled. I called for my husband, Andy, HUNNEEEE! I have LYME!

He really didnt think so.

I banged on my neighbors door, still in my towel, Rachel! Look! Lyme!

She told me no way.

But I knew, and I walked to the ER.

The fresh-faced doc took a quick glance at my shoulder and said it looked like Lyme, and to thank God I wasnt one of those crazy Lyme people that never gets better because they caught it too late.

I filled the CDC recommended 21 days of 200 mg of Doxycycline and fully believed Id be okay.

I followed up with an infectious disease specialist from NYU a few days later, to be safe. He reassured me that I would have no complications since I caught the bite within a week.

I was relieved.

So was I.

What Do You Call It

The CDC, an Atlanta-based federal public agency, doesnt currently use the term chronic Lyme disease, saying it often has been used to describe symptoms in people who have no evidence of a current or past infection.

The Infectious Diseases Society of America doesnt believe in chronic Lyme disease, says Dr. Jose Vazquez, chief of the Medical College of Georgias Division of Infectious Diseases. We recognize Post-Treatment Lyme Disease Syndrome, and so does the CDC.

The Infectious Diseases Society and the International Lyme and Associated Diseases Society agree that antibiotics such as doxycycline are appropriate after the rash appears. But the two associations disagree on how long treatment should last, and what can happen afterward, the Associated Press reported in an article last year.

The Infectious Diseases Society says more research is needed on chronic Lyme, but doesnt recommend long-term antibiotics to treat the condition. ILADS, however, argues on its website that the harm of a continuing Lyme infection outweighs the risks of long-term antibiotic use.

A recent rally at the CDC sought to raise awareness of Lyme sufferers. Geraldina attended the rally, as did Mirenda Campirano, a Texas nurse who says she has been suffering from chronic Lyme for 15 years. She runs an online group called the Lyme Army, a platform for sufferers to speak up, reach out and help others.

Very few of them have indications of the disease, Lyon says.

Why Antibiotics Have Limited Use For Chronic Lyme

Borrelia clears the blood quickly and uses its corkscrew shape to penetrate deeply into tissues where it is protected from antibiotics and the immune system. It can penetrate into cells, give up the corkscrew shape, and live inside of cells, thus gaining protection from the immune system and antibiotics. If confronted with a full antibiotic assault, it rolls up into dormant cyst and rides out the storm until the antibiotics are gone.

Antibiotics work best on highly threatening microbes that grow very rapidly and congregate densely in localized areas in the body . Borrelia grows very slowly in the body and only occurs in very low concentrations in tissues.

Borrelia is a master at blending in with the trillions of other microbes that make up the human microbiome . Overwhelming the host is not its mission it simply needs to scavenge enough resources to survive. Borrelias stealthy ability to lay low makes diagnosis and treatment a real challenge.

Borrelia never occurs alone. There are many stealthy microbes mycoplasma, chlamydia, Bartonella, Epstein-Barr virus , cytomegalovirus , and others that can occupy space in the microbiome. Sometimes they occur as coinfections with the tick bite, but they can already be present in the microbiome, without causing symptoms, when infection with Borrelia occurs.

What Are The Side Effects Of Lyme Disease Treatments

Antibiotics, like all medications, have the potential for side effects. Any antibiotic can cause skin rashes, and if an itchy red rash develops while on antibiotics, a patient should see their physician. Sometimes symptoms worsen for the first few days on an antibiotic. This is called a Herxheimer reaction and occurs when the antibiotics start to kill the bacteria. In the first 24 to 48 hours, dead bacterial products stimulate the immune system to release inflammatory cytokines and chemokines that can cause increased fever and achiness. This should be transient and last no more than a day or two after the initiation of antibiotics.

The most common side effect of the penicillin antibiotics is diarrhea, and occasionally even serious cases caused by the bacteria Clostridium difficile. What to Look for

You may be bitten by a tick in an area of your body that you cant see. But if you can see it, a tick bite can result in a circular bulls eye rash at the site of the bite. It can look like a benign ringworm rash and may appear within 3-30 days, typically before the onset of fever. Not everyone gets the rash. Some patients develop rashes in other areas of the body several days later and have symptoms that can include fever, chills, flu symptoms, fatigue, headache, muscle ache and joint pain.

New Borrelia Burgdorferi Sensu Lato Species Borrelia Americana Spnov Identified In South Carolina And California

On October 21, 2009 the Journal of Clinical Microbiology released the recent research findings of James H. Oliver, Jr., Nataliia Rudenko, Maryna Golovchenko, Tao Lin, Lihui Gao and Libor Grubhoffer. The scientists found a newly described species of Borrelia in South Carolina and California which they have named, Borrelia americana.
